This Privacy Policy (“Privacy Policy”) describes how Relief Health, Inc. doing business as Dorsal (“Dorsal,” “we,” “our,” or “us”) collects, uses, maintains, safeguards, and discloses information that you provide to us when you visit the website located at www.dorsalhealth.com (our “Platform”) and access our Services.

Our “Services” can include (1) pharmacy services, (2) providing health and wellness-related information (“Content”), (3), development and retention of healthcare information for rendering healthcare services, including healthcare provider consultations and pharmacy services, (4) telecommunications and technological support required by healthcare providers employed or contracted by Dorsal Medical Group of California, P.C. or similar affiliated professional entities in their delivery of healthcare services, and (5) administrative support required in the rendering of these services, including scheduling, consultation service billing, and pharmacy service billing.

This Privacy Policy does not apply to information that we procure offline or through any other means. Please read this policy in its entirety. By accessing our Platform and Services, you agree to accept the relationship detailed in this Privacy Policy in its entirety and consent to our collection, use, maintenance, and disclosure of information you provide to us, consistent with the terms outlined in this Privacy Policy. We may amend this Privacy Policy on occasion and your continued use of the Platform and Services following these amendments shall constitute an acceptance of these amendments in their entirety, so we encourage you to check the Privacy Policy on occasion for any updates.

From third parties, such as healthcare providers and pharmacies who we work with in order to render the Services.

Information that you directly provide through our Platform, including information provided when you register for the Services and when we respond to your inquiries related to the Services (for example, queries on billing and treatment).
Information regarding actions and transactions that you execute through our Platform. Your navigational patterns and search queries on our Platform.

Information We Collect Related to Your Online Visit. As you interact with the Platform, we obtain information regarding aspects of your visit and access methodology (e. g. your equipment) using various tracking and data collection methodologies, including:

Usage of our Platform, including logs, location data, date and time of access.
Information pertaining to referring websites, in addition to information related to your online activity over time on other third-party websites and your online behavioral patterns.

Cookies Used by the Platform. Our Platform makes use of temporary (single-session) and persistent (multi-session) cookies. Temporary cookies only last for a particular browser session, disappearing when your browser is closed, and are primarily used for improving system performance. Persistent cookies are stored for longer and are used for tracking traffic patterns. Cookies by themselves are not used by Dorsal for disclosure of individual identity. Cookies instead identify browsers. We use additional tracking technologies in addition to cookies, including various forms of pixel tags, to track Platform usage and visits and also to optimize the Services.

Information Received From Third Party Sources. We may also receive information from third parties who collaborate with us, such as healthcare providers and pharmacies who assist in rendering the Services.

In delivering the Platform and Services, we may structure, analyze, and save your information in various ways, including:

To facilitate communication with you regarding the Platform and Services and your use thereof, including communications sent on behalf of healthcare providers over the course of rendering the Services.
To respond to your queries.
To provide technical support to enhance your user experience.
To identify you and complete various administrative tasks related to your account, such as billing and order fulfillment.
To properly render the Services, including enabling healthcare providers to safely deliver appropriate medical services.
To deliver communications related to our products and service offerings, as well as those of our third party partners, our affiliates, and their third party partners.
To deliver communications related to changes in our Platform or the Services.
To measure the results of advertising we serve to our users and to deliver pertinent advertisements to you.
In the event of a sale, merger, consolidation, change of control, transfer of assets, liquidation, reorganization, we may sell, transfer, or assign to third parties information related to your relationship with us, including any personal information you have provided.
In any case where you have provided your consent where legally required.
To comply with applicable laws.

To facilitate communication with you regarding the Platform and Services and your use thereof, including communications sent on behalf of healthcare providers over the course of rendering the Services.
To members of our group, including any affiliates or subsidiaries.
To pharmacies and other third party entities we partner with in the course of our business, including third parties that assist us in rendering the Services.
To our service providers, including service providers who provide us services related to web hosting and information technology support.
To healthcare providers for (1) scheduling consultations and providing healthcare services, (2) who you communicate with in the course of our provision of the Services, and (3) for other medical and billing purposes, including pharmacy services.
To any buyer or successor in the event of a sale, merger, reorganization, restructuring, divestiture, dissolution, or transfer of all or a portion of Dorsal’s assets, whether as a going concern or as part of a liquidation, bankruptcy, or similar procedure, in which personal information possessed by Dorsal about our users is included in the transferred assets.
To any third parties that we are required to disclose information to in order to comply with applicable laws.
To complete the purpose for which the information was provided – for example, if you ask us in an email to communicate with a third party (such as a healthcare provider), we may share the contents with that third party.
For any additional purposes that you consent to or that we disclose to you when you provide information.
To properly deliver the Services, including accomplishing billing and collection tasks.
To safeguard the property, safety, and rights of Dorsal, our users, or others, including sharing information with third parties for the purposes of credit risk management and fraud protection.

Your Input in Our Usage and Disclosure of Your Information

We endeavor to offer you the ability to provide input related to our use of the personal information you provide to us, and we have developed methods for providing you control over aspects of our use of the information.

Tracking and Advertising. You have the ability to set your browser to disable browser cookies and notify you of cookie usage. Should you disable cookies, certain components of the Platform may not function as intended. We may use Google Analytics to track your interaction with the Platform. Should you want to disable Google Analytics’ ability to track your interactions, then you can opt-out through your web browser (https://tools.google.com/dlpage/gaoptout); you can also opt-out from Google Display Network and Google Analytics for Display Advertising using Google’s Ads Settings located at https://www.google.com/settings/ads.

Promotionals. If you do not wish to have us use your contact information in the promotion of our products and service offerings, you may send us a return email to any promotional email asking that we do not include you in future email distributions, or you may contact us at the address below.

We do not control the activities of third parties with regards to the collection and use of your information in the delivery of advertisements and promotions.

Modification of Information

To request access to, to correct, or to delete any personal information you provide to us, you may contact us at the address below. If you ask us to delete your personal information, we must also delete your user account. In this event, healthcare providers and other affiliates may still have the right to retain information, subject to applicable laws and their own retention policies. We cannot modify your information if we believe that such a modification would be incorrect or would result in a violation of the law. We will also delete information if we determine that it has been unlawfully processed or if it becomes necessary to do so in order to comply with applicable law.

Security

We place strong emphasis on data security and we have implemented security procedures and measures to protect your personal information from unauthorized access, use, disclosure, or unintended loss. Where you have chosen or where we have provided a password to access the Platform, you retain the responsibility of maintaining the confidentiality of the password. You may not share your password with others.

Although we strive to protect your information to the best of our ability, we cannot guarantee the security of your information, and any transmission of information is done at your own risk.

Third Parties

We are not responsible for the information collection standards, security standards and data privacy standards of any third parties, including healthcare providers who we collaborate with and the manufacturers of any electronic device you use to access the Platform and Services, and this Privacy Policy does not address the practices of any third parties.

Even though Dorsal may not be a “covered entity” under HIPAA, healthcare providers who you engage with as part of the rendering of the Services may be “covered entities” and as such may be subject to the provisions of HIPAA at times. Your accessing of the Platform and Services and your acceptance of this Privacy Policy incorporates your consent and acceptance to the Dorsal Professionals Notice of Privacy Practices included on the Platform. This notice details the manner in which your healthcare providers may use and disclose your PHI. Dorsal’s collection, use, and disclosure of your PHI, on behalf of your healthcare providers, will be consistent with the Dorsal Professional Notice of Privacy Practices except where you explicitly consent to additional uses and disclosures.

Use by Minors

If you are not yet 18 years of age, we must obtain the consent of your parent or legal guardian to this Privacy Policy prior to your use of the Platform and Services. We will determine the acceptable manner of consent at our sole discretion, and if we assess at any time that the consent is inadequate, we reserve the right to not provide you access to the Platform and Services. If you are a parent, legal guardian, or other representative of a minor child not yet 17 years of age, you may use the Platform and Services on behalf of the child. We do not knowingly collect information for minor children unless we have obtained adequate consent.

Privacy Policy Modifications

We reserve the right to modify this Privacy Policy at any time. The last date of revision to this Privacy Policy is identified at the top of this page. Your continued use of the Platform following any amendments will be deemed an acknowledgement and acceptance of these amendments, and you are responsible for occasionally reviewing any updates to the Privacy Policy.
